When the federal government began to push hospitals to reduce their readmission rates, some claimed that wasn’t feasible or practical, especially when they were operating in poverty prone regions. With technology, though, there has been a more fervent push to help men and women through more accurate and diligent monitoring of their vitals at home and for those who may not be able to get to their doctor for follow-up care, or for those who may not know the right things to do, this can be beneficial.
Valley Home Care in New Jersey, has been using TeleHealth services. This is a system that allows patients to be monitored at home and a doctor tracks their progress remotely. A visiting nurse would check in on the patient as required -whether daily, weekly, or at some other interval- and enter their vitals into a system. Some patients may also have their vitals monitored through this technology and if the doctor notices any abnormal patterns that cause concern, they can address them immediately, rather than waiting until an emergency forces the patient back to the hospital.

Valley Home Care has achieved a hospital readmission rate of just 8.2% for 416 at-risk patients. Compared to the national average of 16.7% and the New Jersey average of 16.9%, it’s an impressive early track record.
Numerous technology firms have been devoted incredible resources into developing better systems that will allow doctors and hospitals to remotely monitor their patients to help reduce these rates and offer better access to care for the elderly and poor patients who might be left out of the system otherwise.
Technology is proving to be a valuable asset in the fight to reduce readmission rates, but having the right professionals for in home care support is just as tantamount to long-term success in reducing these rates.
